Job Description Job Description Care Management Coordinator Location: Harrisburg, PA (Onsite with Community Travel) Pay Rate: $20.00/hour Employment Type: Temporary / 6-month Project-Based (Potential for Hire) Mileage Reimbursement: Paid at the Federal Mileage Rate Parking: Free parking Job Overview Abel Personnel is seeking a reliable and thoughtful Care Management Coordinator to support a community-based care management and protective services program in the Harrisburg area. This role is well-suited for individuals who are comfortable working with people, gathering information, and supporting care planning through both phone-based and in-person interactions. The Care Management Coordinator will assist with care plan reviews, client outreach, case documentation, and coordination of services, balancing office-based responsibilities with limited community travel. Key Responsibilities Accept and screen incoming calls, referrals, intakes, and walk-in inquiries Conduct telephone and in-person interviews with clients and individuals involved in active cases Review and support the ongoing management of Care Plans, identifying needs and coordinating appropriate resources Travel into the community a few times per week to meet with clients and review Care Plans in person Place outbound calls to individuals involved in care coordination or case investigations Think on your feet by asking appropriate follow-up questions and adjusting conversations based on client or collateral responses Accurately document case activity and client interactions in the agency’s computerized client information system Maintain professional relationships with community agencies, providers, and support networks Follow agency policies related to confidentiality, documentation, and client care Required Qualifications Bachelor’s degree in Social Work, Sociology, Psychology, Gerontology, Human Services, or a related social science or an equivalent combination of education and relevant experience Strong communication and interviewing skills, both by phone and in person Ability to process information quickly, problem-solve, and adapt questioning in real time Comfortable working independently while managing multiple tasks and priorities Proficiency with basic computer systems, data entry, and electronic case documentation Ability to work respectfully with individuals experiencing physical, emotional, or social challenges Valid driver’s license and ability to travel locally (mileage reimbursed at the federal rate) Work Environment & Expectations Onsite office environment in Harrisburg with regular local community travel Combination of desk work, phone-based outreach, and field visits Interaction with clients who may be distressed or navigating complex situations Requires professionalism, sound judgment, flexibility, and empathy Why Work with Abel Personnel $20.00/hour pay rate Federal mileage reimbursement for required community travel Opportunity to gain experience in care management, protective services, and community-based services Support from a well-established, locally respected staffing firm Apply Today If you’re looking for meaningful work that blends office coordination with community engagement and client interaction, we encourage you to apply through Abel Personnel.
