Complete timely and accurate clinical documentation in compliance with facility and regulatory standards. Develop and implement individualized treatment plans in collaboration with the interdisciplinary team. Meet productivity standards while maintaining high quality of care. Participate in patient/family education and discharge planning as appropriate. Requirements Valid MD state licence as a Physical Therapist (PT) required. Graduate of an accredited programme in your discipline. Minimum 1 year of clinical experience in SNF / post-acute care preferred. Current BLS/CPR certification.
