Assess patient needs and create detailed plans for care and rehabilitation. Provide counseling and psychosocial support to patients and their families. Coordinate with healthcare professionals to implement comprehensive care plans. Maintain patient records and document all patient interactions and plans. Advocate for patient access to community resources. Educate patients and families on healthcare systems, chronic disease management, and lifestyle decisions. Participate in training and professional development activities. Strong interpersonal and communication skills. Ability to handle sensitive information confidentially. Knowledge of healthcare systems and resources. Crisis intervention skills. Proficient in clinical documentation and record keeping. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Clinical Social Worker (LCSW) preferred. Valid state clinical social worker license.
