Key Responsibilities: Deliver physical therapy services to patients
in their homes, including assessments, treatment plans, and
interventions tailored to each individual’s needs. Evaluate the
home environment and recommend modifications or resources to
improve safety and mobility. Monitor patient progress and
communicate updates or changes in condition to physicians.
Collaborate with caregivers, nurses, and other healthcare
professionals to ensure coordinated, comprehensive care. Refer
patients to additional home health services when appropriate.
Participate in discharge planning and ensure continuity of care.
Supervise Physical Therapy Assistants in accordance with company
policies and state regulations. Qualifications: Bachelor’s or
Master’s degree in Physical Therapy from an accredited program.
Current Physical Therapy license in the state of Pennsylvania.
Minimum of 2 years of experience as a Physical Therapist; home
health or community-based care experience preferred. Valid CPR
certification. Reliable transportation and a valid driver’s
