compassionate service. We are currently recruiting for Integrated
Case Managers to join our Community Services team at Disston
Elementary School. This position is full-time and on site at
Disston Elementary School Minimum starting pay - $21.20/hr (approx.
$44,000/yr) All CORA team members are expected to uphold the

growth In addition, as an Integrated Case Manager your duties will
include: Providing integrated case management services to young
people and their families to increase family functioning &
engagement in school activities, and students’ attitudinal
connectivity & relationship to school with the goal of increasing
student attendance and achievement Collaboratively supporting
schools, students and families to provide resources and linkage to
services for families experiencing socioeconomic and health
stressors; making and monitoring referrals to remove any barriers a
student/family may face to equitably accessing programs/resources;
supporting families to establish/maintain household stability;
submitting requests for use of emergency funds assistance as
applicable Maintaining a caseload of twenty-five or more students
and families; completing weekly contact with families on assigned
caseload including regular home visits as needed to assess
families’ needs for services and developing service plans to remove
barriers to regular school attendance Engaging students and
families in a collaborative relationship that will enhance the
potential for students’ completion of service with positive
outcomes; involving families at the school and helping them
navigate school-based resources to increase parent engagement;
planning, coordinating and implementing family forums on a monthly
or quarterly basis Partnering with the Attendance Case Manager by
participating in SAIC, when necessary, and working with the
school’s attendance team to ensure barriers to regular school
attendance are removed for families experiencing truancy Planning,
coordinating, and implementing weekly, monthly, and quarterly
attendance incentives and events in partnership and collaboration
with the school attendance team and CORA case management leadership
team Adhering to CORA’s and regulatory agencies’ guidelines
concerning caseload requirements, service delivery, accountability
and confidentiality; maintaining accurate and timely documentation
of services provided to students/families; participating in all
necessary interdisciplinary functions and meetings; adhering to
training requirements established by CORA or other regulatory
entities; attending regular supervision and department meetings
Proactively seeking and maintaining awareness of community
resources and facilitating student/family access to appropriate
resources and services necessary to support and supplement service
goals Completing other tasks, projects and assignments as directed
MINIMUM QUALIFICATIONS: Bachelor’s (or master’s) degree in social
work, human services or related discipline 2 years’ work experience
in a social services capacity; treatment or intervention services
experience preferred Working knowledge of delinquency prevention
concepts and the effects of substance use and abuse Demonstrated
case management experience preferably with young people and
families Outstanding communication and interpersonal skills;
ability to establish and maintain positive, productive working
relationships internally and with those external to CORA (students,
families, teachers, school administration, etc.) Valid driver’s
license required Proficient with Office 365 applications CORA
