Job Description:

General Summary of PositionResponsible for the coordination of patient care for stroke patients. Educates/provides information, supports patients, and facilitates treatment plans. Functions as a patient advocate, guides patients and families throughout care. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital's philosophy, policies, procedures and standards.

Primary Duties and Responsibilities

• A. Performs the intake, assessment/triage, coordination and navigation of stroke patients .
• B. Serves as a liaison for referring physicians seeking information, or access to the system.
• C. Screens patients for appropriate information and manages the acquisition of pertinent medical records for initial appointments.
• D. Responsible for the planning and implementation of the patient evaluation process.
• E. Facilitates the evaluation of patients by interfacing with referring physicians and MGUH services.
• P. Identifies needs and develops patient education and support material.
• Q. Provides community education and outreach.
• R. Assumes other duties and responsibilities that are appropriate to the position and area. The above responsibilities are a general description of the level and nature of the work assigned to this classification and is not to be considered as all-inclusive.

  Minimum Qualifications Education

• Bachelor's degree Nursing required
• Master's degree Nursing preferred
  
  
  Knowledge, Skills, and Abilities
  • Must have clinical maturity and experience, excellent interpersonal skills, ability to work collaboratively with an interdisciplinary team of health care providers. Effective oral and written communication skills. Position requires a high level of advocacy and problem-solving ability. Ability to multitask and organize advocacy and support for a large and varied group of stroke patients and their families. Knowledge of community resources is helpful.
    This position has a hiring range of $87,318 - $157,289
